What is PHP Survey?
PHP Survey is a php web script,which is easy to install (not using any MySQL database and not requiring any database configuration) allowing to create online surveys like customer satisfaction or product evaluation or other surveys with the questions and answers you may need in them. It comes with an user-friendly administration panel allowing manage the different settings,create your own custom surveys with different field types,check the completed surveys and others.

Using the functionality in the administration panel,you could easily add any number of questions you wish in and choose the field types (between text boxes,check boxes,radio buttons,text areas and drop down menus) . For the different questions you can enter a list of predefined answers or let the users enter their own answer / text.
Manage the surveys,add them in other sites with iframe or send your users a direct link
You can edit the different questions of the saved surveys,change their types or add new questions for the surveys. The surveys can be also easily added in other sites by pasting an iframe code or you can also send a direct link to the users,to fill the survey on the site / server on which the script is hosted.
Edit the configuration options and site settings
The functionality to edit the different configuration options and settings allows to customize the site colors,the fonts,which are used when showing the survey questions,manage the email notification settings,SEO urls and others.
Check the completed surveys online or have them sended to you by email
The completed surveys can be checked online from the administration panel coming with the script or they can also be sent to an email address set by the administrator.
Create anonymous surveys or ask the users for their contact details
When creating a new survey,you can set if the survey should be anonymous or the user should be asked for his contact details (name,email and phone). The anonymous surveys only show the survey questions / answers without asking the user for his name or other contact information.
It's really easy to install and no MySQL database is used
PHP Survey doesn't use any MySQL database-the information is stored instead in local XML files. This makes also the installation really easy because to install it on your server or hosting package,it's only necessary to unzip it and upload the files in the folder you prefer (no need to install previously a database or create database configuration files).
